Thoughtful, flexible, and compassionate support for daily living.

How we Support Families

Non-Medical Home Care

Trinity Care and Consulting provides non-medical care coordination and administrative support for older adults and individuals requiring daily assistance. We work with a curated network of independent caregivers and remain closely involved to support communication, continuity, and alignment throughout the care relationship.

Our Services Include:

  • Conducting in-home intake visits to understand the client’s needs, preferences, and household dynamics, in collaboration with the client and their support team.

  • Facilitating thoughtful introductions to independent caregivers whose experience and availability align with the client’s care needs.

  • Coordinating one-to-one non-medical support provided by independent caregivers, which may include assistance with activities of daily living as agreed upon between the client and caregiver.

  • Supporting routine structure and non-medical reminders as part of the overall care arrangement.

  • Assisting with coordination around light household support and meal preparation, as determined between the client and caregiver.

  • Supporting communication around mobility routines and daily activity, consistent with existing care plans and within a non-medical scope.

  • Maintaining clear, ongoing communication with family members and the broader support team.

  • Assisting with scheduling communication and continuity planning to support consistent care arrangements when possible.

  • Offering flexible care coordination for a range of needs, from companionship to extended one-to-one support.

  • Remaining engaged through regular check-ins to support alignment, continuity, and overall satisfaction for both clients and caregivers.

Hourly and Live-in Support

Trinity Care and Consulting coordinates hourly and live-in one-to-one care arrangements designed to support a wide range of needs while prioritizing continuity, fit, and discretion.

Our focus is on creating thoughtful alignment between clients and independent caregivers—particularly for ongoing live-in arrangements where consistency and familiarity are essential.

For clients who benefit from greater flexibility, Trinity supports the coordination of customized hourly care schedules, shaped by the client’s preferences and the caregiver’s availability.

Both hourly and live-in arrangements support non-medical, one-to-one care, with services tailored to the individual and defined through the relationship between the client and caregiver. Throughout the care relationship, Trinity remains involved as a coordination and administrative partner, supporting communication, continuity, and overall alignment.

During hospice and end-of-life care, Trinity Care and Consulting provides thoughtful care coordination and administrative support to help families navigate this deeply personal time with clarity and calm.

We work alongside the client, their family, and the existing hospice care team to support communication, continuity, and daily comfort, while honoring the role of medical providers. Non-medical support is provided by independent caregivers and is tailored to the needs and wishes of the client and family.

Trinity’s role is to help manage the practical aspects of care coordination—so families are free to remain present, connected, and focused on meaningful moments together.

Throughout this stage, we remain available as a steady, compassionate point of support, helping to maintain dignity, comfort, and a gentle sense of continuity.

Caregiver Support & Alingment

At Trinity Care and Consulting, we believe exceptional care is built on mutual respect, clarity, and strong relationships. Independent caregivers who participate in Trinity’s network are valued as trusted partners in the care experience.

We support caregivers through clear communication, thoughtful coordination, and access to relevant resources, helping foster alignment and continuity within each care relationship. Our role is to provide structure around the caregiving experience—so caregivers feel informed, supported, and able to focus on their work with confidence.

By maintaining open communication and a respectful, relationship-based approach, we aim to support long-term alignment and satisfaction for both caregivers and the families they serve.
